Recent studies indicate that a combination of early rhythm control therapy for AF and a healthy lifestyle can significantly lower the risk of developing all subtypes of dementia in patients newly diagnosed with AF [1]. For older adults with AF, direct-acting oral anticoagulants (DOACs) are often recommended to prevent stroke [2]. An interdisciplinary approach, involving various medical specialists, is considered appropriate for managing AF and its associated risks [8].
Modifiable Vascular Risk Factors for Brain Health
Several well-established vascular risk factors contribute to both stroke and dementia. These include high blood pressure (hypertension), smoking, obesity, diabetes, high cholesterol (hypercholesterolemia), and low physical activity [6, 7]. - Blood Pressure Management: While lowering blood pressure is generally beneficial, a target systolic blood pressure below 120 mmHg might pose risks for older patients with stiff arteries [2]. Remote patient monitoring of blood pressure is an emerging tool for better management [3].
- Cholesterol Control: Lipid-lowering therapies, such as statins, can be particularly beneficial for older patients, potentially offering even greater advantages than for younger individuals [2].
- Diabetes Management: Effective control of blood sugar is vital. Newer medications, such as glucagon peptide-like protein-1 receptor agonists, are increasingly used in stroke prevention strategies [3].
Lifestyle: A Cornerstone of Prevention
Adopting and maintaining healthy behaviors is fundamental to preventing both dementia and stroke [4]. These behaviors encompass a broad range of choices:
- Diet: Eating a balanced diet is important. The interaction between the intestinal microbiome, age, and kidney function also has significant dietary implications for older adults [2].
- Physical Activity: Regular exercise is a key component of a healthy lifestyle [4].
- Smoking and Alcohol: Avoiding smoking and limiting alcohol consumption are critical for reducing risk [4, 6].
- Sleep: Ensuring adequate and quality sleep is important for overall brain health [4].
- Stress Management and Social Engagement: Managing stress, maintaining social relationships, and having a sense of purpose in life are also recognized as healthy behaviors linked to brain health [4].
Despite widespread knowledge about these modifiable risk factors, many individuals struggle to consistently practice these healthy behaviors [4]. Lifestyle interventions have also been shown to positively impact atrial fibrillation, further reducing associated risks [8].
Beyond Common Risks: Emerging Factors and Prevention Strategies
While traditional risk factors are well-known, research continues to uncover additional contributors to stroke and dementia risk. These include chronic kidney disease, obstructive sleep apnea, migraine with aura, certain working environments, genetic factors, and even air pollution [6].
Advances in prevention also include new risk prediction algorithms that incorporate social determinants of health and aim to remove racial biases, offering a more comprehensive approach to identifying individuals at risk [3]. Additionally, B vitamins, which can lower homocysteine levels, may play a role in stroke prevention [2].
Stroke Prevention as Dementia Prevention
The strong connection between stroke and cognitive decline underscores the importance of stroke prevention as a primary strategy for preserving brain health [2]. Recurrent ischemic stroke, in particular, significantly increases the risk of further cognitive decline [5].
For individuals who have already experienced an ischemic stroke, secondary prevention is essential to lower the risk of future strokes. Interestingly, patients with dementia who have an ischemic stroke may receive higher rates of planned medication initiation for secondary prevention at hospital discharge compared to those without dementia [5]. Long-term monitoring for conditions like atrial fibrillation is also a crucial aspect of ongoing stroke prevention efforts [3].
