Understanding Statins and Cholesterol

Who Should Consider Statin Therapy?
Statins are recommended for a broad range of individuals to reduce cardiovascular risk. This includes patients with established atherosclerotic cardiovascular disease (ASCVD), such as coronary artery disease, peripheral artery disease, or cerebral atherosclerosis [6]. As mentioned, those who have experienced an ischemic stroke or TIA of atherosclerotic origin are also strong candidates for statin therapy [1].
Dyslipidemia, or unhealthy lipid levels, is a significant risk factor for stroke, ranking after conditions like hypertension, diabetes, and smoking [4]. Addressing Treatment Gaps and Misconceptions
Despite clear guidelines recommending aggressive LDL-C lowering in patients with ASCVD, significant gaps in treatment persist in clinical practice [6]. Many patients who could benefit from lipid-lowering therapy are not receiving it, or their LDL-C levels remain above recommended targets, often higher than 70 mg/dL [6]. This highlights the importance of ongoing discussions with healthcare providers about managing cardiovascular risk factors.
While statins are generally well-tolerated, potential side effects such as new-onset diabetes, muscle pain, and liver toxicity are possible, though often manageable [4]. It is crucial to discuss any concerns with a doctor. Another point of confusion can be the "lipid paradox," where some studies in statin-naïve patients with a first heart attack show a paradoxical association between low admission LDL-C and worse outcomes [8]. It does not negate the benefits of statin therapy in appropriate patients [8].
