Educational initiatives have shown success in improving this awareness [4]. However, even with increased awareness, the varied presentation of stroke symptoms can complicate diagnosis.
For instance, certain conditions can present with symptoms that strongly resemble a stroke. Hemiplegic migraine, a rare type of migraine, can cause reversible unilateral motor weakness, along with visual, sensory, or speech disturbances, often leading to urgent stroke evaluations due to the striking similarity in symptoms [8]. Similarly, an acute ischemic stroke might be the first indication of a 'silent' myocardial infarction (heart attack), especially in individuals with existing conditions like hypertension and type 2 diabetes. This highlights the need for a comprehensive diagnostic approach when the cause of a stroke is not immediately clear [6].
Stroke in Younger Individuals and Trauma Patients
While often associated with older adults, stroke can affect people of all ages. Cervical artery dissections, which involve tears in the carotid or vertebral arteries in the neck, are a common cause of stroke in children, young adults, and individuals who have experienced trauma, despite their overall low incidence [1, 2].
Symptoms of these dissections can include headache, neck pain, and dizziness. These symptoms are frequently encountered in emergency departments, but they may not be easily apparent in trauma patients who are less responsive, or they might not be immediately recognized as being related to a dissection [1, 2]. The Importance of Timely Medical Care
When a stroke is suspected, time is critical. Emergency medical professionals are trained to recognize and respond to potential stroke events. For certain conditions that increase stroke risk, timely intervention is paramount for prevention. For example, best practice guidelines recommend that symptomatic patients requiring carotid endarterectomy (CEA)—a surgical procedure to remove plaque from the carotid arteries—undergo the procedure within two weeks to optimally prevent stroke. However, data from Ontario in 2013 indicated that only a small percentage of eligible patients met this target. Implementing 'fast-track' systems has been shown to improve the timeliness and quality of care for patients needing urgent CEA [3].
Understanding Post-Stroke Complications and Treatment Nuances
Stroke can also lead to other neurological issues, such as seizures. Acute symptomatic seizures can occur within the first seven days after a stroke and are considered provoked events. While seizures after a stroke are a concern, current evidence does not support routine primary preventative antiseizure medication for all stroke patients. However, if clinical or electrographic seizures are documented, appropriate treatment is necessary, and established post-stroke epilepsy often requires long-term, individualized therapy [5].
Regarding treatment for acute ischemic stroke, research continues to refine best practices. A meta-analysis of randomized controlled trials found that initiating antihypertensive therapy within 48 hours of symptom onset did not significantly improve all-cause mortality or functional dependency compared to placebo or usual care [7]. This highlights the complexity of stroke management, where treatment decisions are carefully weighed based on the latest evidence.

Given the complexities of stroke symptoms, its potential mimics, and the critical window for intervention, immediate medical attention for any suspected stroke symptom is non-negotiable. While medical professionals work to apply advanced screening criteria, imaging options, and appropriate treatments, public awareness and prompt action remain the first line of defense against the devastating effects of stroke [1, 2].
